About the Role:
The Adult Hospital Social Worker provides comprehensive social work services to adult patients and their families. This role involves assessing patient needs, developing care plans, providing counseling, and coordinating with healthcare professionals to ensure holistic care. The social worker will advocate for patients’ rights and support them through their healthcare journey.
What will your responsibilities be?
Conduct psychosocial assessments to identify patient needs and develop individualized care plans.
Provide emotional support and counseling to patients and their families dealing with illness, hospitalization, and treatment.
Collaborate with medical staff, nurses, and other healthcare professionals to ensure integrated care.
Advocate for patient rights and access to necessary resources and services.

Qualifications & Experience:
To be successful in this role you must have,
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
