Community Placement Executive
Permanent, Full Time
Field based - East Midlands
£28,000-£32,000 per annum plus mileage allowance

Must drive and have access to a vehicle

We are looking for a friendly proactive and driven individual to join our team, building strong relationships, supporting healthcare partners and driving regional growth. This is a fantastic opportunity to combine commercial instinct with meaningful impact - helping to ensure that people can access the care and support they need.

💡 About the Role

In this role, you will:

* Develop and strengthen referral partnerships to support increased access to care for elderly patients across your assigned area.

* Develop and maintain relationships with care providers including hospitals, GPs & social workers.

* Maintain accurate records of referral activity and provider engagement.

* Ensure providers meet quality and compliance standards.

* Deliver exceptional customer service to clients and providers.

This is a role where your ability to build meaningful relationships, spot opportunities and understand customer needs will impact people’s lives and make a real difference.
