Bury St Edmunds Primary Care Network (PCN) is looking for a motivated and compassionate care coordinator to join our growing team on a full-time basis. This is an exciting opportunity to play a key role in supporting patients across our practices by ensuring they receive the right care, at the right time, from the right service. As a care coordinator, you will: Work closely with GPs, nurses, and other healthcare professionals to support patients with complex health and social care needs. Help patients navigate the health and care system, ensuring they are connected with the most appropriate services. Act as a point of contact for patients and carers, providing personalised support. Coordinate care for people with long-term conditions, frailty, and those requiring proactive management. Contribute to improving patient experience and outcomes within the PCN. About the Candidate We are looking for someone who is: Highly organised with strong communication skills. Able to work independently and as part of a team. Compassionate, approachable, and patient-focused. Confident using IT systems and managing information. Experience working in the healthcare sector, care, or community support roles. About Us Bury St Edmunds Primary Care Network consists of five surgeries and serves some 66,000 patients from the town and surrounding villages. Our aim is to create a system where member practices work closely together in collaboration with health and social care, voluntary sector, community groups and local people, to deliver inclusive and personalised care. Bury St Edmunds Primary Care Network is a growing PCN with already existing roles including, care coordinators, social prescriber, clinical pharmacists, paramedics, health and wellbeing coaches and pharmacist technicians that support GPs and other clinical teams to enhance patient services in the local community.
