Are you an experienced community therapist? Do you have an interest in projects and Quality improvement?

Now is an exciting time to join the Adult Community Rehabilitation Team. We are looking for a highly motivated, skilled, experienced Occupational Therapist, Physiotherapist or Speech and Language Therapist to work as the lead for screening within the team.

The Adult Community Rehabilitation Team (ACRT) is a forward-thinking service that provides an interdisciplinary rehabilitation service to adult clients. The team is split into two specialist branches, physical conditions and neurological conditions consisting of Physiotherapy, Occupational Therapy, Speech & Language therapy, Clinical Psychology, Dietetics and Rehabilitation Assistants.

The team is supported by a Consultant Allied Health Professional, Consultant Neuropsychologist and a Consultant Neurologist. Therapy intervention is goal directed and flexible. The service is essentially community based, and clients can also be seen in on-site facilities across the borough.

You will work alongside colleagues in a modern open plan canal side office with roof terrace. Main duties of the job

You must have expertise working with clients in a community setting. You will lead, manage and co-ordinate the screening team which involves close working with other community therapy colleagues and referrers.

You will have support from an administrator, other ACRT colleagues, the Head and Deputy Heads of Service to ensure the smooth running of ACRT's 'front door'.

The role involves reviewing and streamlining the screening service within ACRT and will have be expected to both lead and support wider innovative quality improvement projects focused on improving client outcomes and community therapies flow.

* Lead the screening pathway within ACRT, screening referrals and allocating cases to appropriate care pathways in timely manner.
* Liaising with primary, secondary, health and social care providers, to case manage individuals into appropriate service or declining referrals, where necessary.
* Promote independence for service users, enabling them to live in the least restrictive environment whilst ensuring they receive the necessary care and treatment.
* If required, carry out community assessments of clients referred, to decide on appropriateness for the service.

Person Specification


Qualifications

* Knowledge of person centred assessment practice including: medical, psychological, social and physical aspects of the service user
* Professional qualification (Occupational therapy, Physiotherapy, SLT, Dietetics)


Skills

* Work on own initiative and work un supervised
* Converse with and listen to the views of service users and their families
* Recognise the needs of client/relatives carers
* Ability to analyse efficacy of processes and recommend /manage changes
* Demonstrates excellent communication skills with all disciplines and agencies Clear and concise verbal communication


Experience

* Experience of working in a multi-disciplinary environment
* Experience at a Band 6 level or above
* Experience of working across organisational boundaries within health, social care or mental health


Knowledge

* An understanding of the current issues in the delivery of NHS and Social Care, especially community based services
* An understanding of the structure for the delivery for primary health care services and of current, local / national developments
