Are you an exceptional clinical pharmacist ready for a new challenge? Our pharmacy team needs capable and enthusiastic individuals with the right values to be a key part of our dynamic teams in the following areas: Hepatology (including liver transplant); Renal Medicine/Transplantation; Surgical specialties including Acute Surgery, Upper GI/Pancreas; Surgical Oncology, and Urology. The posts comprise a continuous, one year clinical rotational programme, designed to steer you through the Advanced Level Framework.
You will have clinical pharmacy experience and a diploma. If you are not already a prescriber, we would support you to gain this qualification.
Post holders will provide medicines optimisation services to patients whilst integrating into the multidisciplinary team day-to-day. As part of our support to Abdominal Medicine & Surgery CSU we also provide clinical pharmacy services to Gastroenterology and Colorectal Surgery. We also offer experience in anticoagulation clinics, including warfarin bridging, and Aseptic dispensing in our busy Aseptic & Production Unit.
The way in which our team works supports close working with specialty teams and ward round attendance to make a difference to patient care through MDT working.
Main Duties of the Job
Pharmacy Services at LTHT is nationally recognised as an exemplar service. The CSU responsibilities span the provision and use of medicines across all patient pathways.
You will receive support to develop from our advanced clinical pharmacists, and appropriate training. You will offer ideas and collaborate to improve patient care and experience, and will be a key part of the implementation of service developments.
Your role will include supervising rotational junior pharmacists and supporting pharmacy technicians in their emerging clinical roles.

Job Responsibilities
 * Provide specialist clinical pharmacy service to patients and the multidisciplinary team within each rotation, as part of a team of pharmacists working in that speciality being professionally accountable for their actions.
 * Provide drug‑related advice and information to patients, consultants and other health‑care professionals within a Clinical Management Team.
 * Monitor the clinical use of drugs in patients.
 * Advising medical and nursing staff how to optimise drug therapy.
 * Reporting prescribing and administration near misses, and risk management issues.
 * Provide education to healthcare professionals within the speciality.
 * Participate in the training of junior pharmacy staff within the speciality.
 * Support the Lead Pharmacist for the area with the production and implementation of drug‑related policies and procedures, with the production of financial reports and with horizon scanning activities.
 * Participate in operational service commitments to dispensaries, Medicines Information and aseptic services.
 * The post holder will be required to work evenings, weekends and Bank Holidays on a rotational basis and participate in the on‑call from home pharmacists rota.

Qualifications
 * Masters qualification in Pharmacy, or equivalent qualification, plus completion of pre‑registration training.
 * Pharmacist Registrant of the General Pharmaceutical Council (GPhC)
 * Postgraduate Diploma in Clinical Pharmacy or equivalent experience or qualification
 * Member of a relevant Professional Development Body e.g. RPSGB, BOPA, NPPG
Desirable Qualifications
 * Non‑Medical Prescriber
Experience
 * Moderate or significant post registration experience as a practising pharmacist.
 * The ability to interpret information and provide advice.
 * Knowledge of hospital pharmacy practice.
 * Legal requirements of handling medicines.
 * Broad clinical pharmacy knowledge.
Desirable Experience
 * Experience of Aseptic dispensing.
