Job Summary
Hours of Work: 37.5 hours per week including one weekend every four weeks. This role combines management responsibility for the Greenwich District Nursing Team based at Plumstead Health Centre. The post holder will lead a team of nurses and support staff managing care for a defined caseload within community and residential settings. Responsibilities include autonomous assessment, diagnosis, prescribing, referral, coordination of care with partner agencies, teaching, appraisal and continuous improvement of clinical services.

Job Description
* Management Responsibilities: Lead and develop the District Nursing Team, manage staffing, recruitment, appraisal, and performance. Ensure statutory and mandatory training, DBS, and NMC registration are maintained for all staff.
* Clinical Leadership: Autonomously assess and plan care for patients in home, residential and clinical environments. Use advanced practice and prescribing to manage complex conditions and coordinate with GP & specialist teams.
* Communication: Effectively communicate complex information to patients, carers and multidisciplinary teams; support patient education and end‑of‑life discussions; document accurately in patient-held records.
* Service Development: Monitor caseloads, conduct proactive audit, embed core values, and lead service improvement initiatives with senior management.
Person Specification
Essential Education / Qualifications
* BSc Specialist Community Practitioner Qualification in District Nursing
* Evidence of managing a District Nursing Team at Band 6 level for a minimum of 2 years
* Accredited Non‑Medical Prescribing Programme
Desirable Education / Qualifications
* Mentor Course or recognised alternative (e.g. HCAI Level 2)
Experience
* Proven senior nurse clinical experience at band 6 or equivalent in an acute or community setting
* Extensive experience assessing and providing care to people with complex and long‑term conditions
Skills / Abilities / Knowledge
* Excellent verbal and written communication skills, ability to manage conflict constructively
* Proactive service improvement mindset and evidence‑based clinical practice
* Commitment to professional development and willingness to acquire new competencies
