Practice Nurse - Park View Health Partnership

We are seeking a skilled, compassionate, and experienced Qualified PracticeNurse to join the Mid Sussex Division of Modality Partnership on a part-timebasis 24 - 30 hours per week based at Park View Health Partnership.This is far more than a traditional nursing role you will play a vital part indelivering high-quality, evidence-based care, preventative health services, andsupporting innovation in primary care.

Main duties of the job

Thisrole focuses on the delivery of high-quality,evidence-based nursing care to patients with chronicconditions, alongside the provision of preventativehealthcare to the wider practice population. You will use yourclinical expertise and professional judgement to assess, plan, implement, andevaluate patient care, demonstrating criticalthinking, accountability, and excellence in every interaction.Working innovatively and flexibly, you will contribute to service improvementand support the effective delivery of care across the practice. The rolerequires commitment, adaptability, and the ability to work resourcefully aspart of a multidisciplinary team, ensuring safe, effective, and timely care inline with our CARE values.

Vaccinations: Mandatory vaccination evidence is required for this role,including Hepatitis B, MMR, and Varicella.

Skills

* Clinical leadership skills with the ability to support and guide others.
* Clinical skills including cervical cytology, immunisation and vaccination and minor surgery.
* Change-management skills with the ability to support patients in making lifestyle changes.
* Strong communication skills, both written and verbal.
* Ability to communicate sensitive or difficult information to patients and their families.
* Negotiation and conflict management skills.
* Ability to provide teaching, supervision, and mentorship in a clinical setting.
* Competent IT skills and confidence using digital systems.


Knowledge

* Knowledge of managing patients with long-term and chronic conditions.
* Understanding of accountability within your own role and the roles of others in Primary Care.
* Knowledge of effective health promotion and preventative care strategies.
* Awareness of local and national health policies and priorities.
* Understanding of the wider health and care system.
* Knowledge of clinical governance principles within Primary Care.
* Understanding of patient group directions and related policies.
* Knowledge of public health principles and population health.
* Ability to recognise factors affecting health within the local community.
* Knowledge of local public health issues and challenges.

Qualifications

* Registered Nurse qualification with a minimum of two years post-registration experience.
* At least one year of experience in General Practice or community nursing.


Experience

* Experience in nurse-led management of chronic conditions.
* Experience implementing clinical guidelines and protocols.
* Experience in audit activity and quality improvement initiatives.
* Clinical supervision training and experience.
* Chronic disease management experience beneficial, but not essential.
