Our services in Stoke-on-Trent are based in the Meir Heath area where we provide 24 hour supported living facilities providing care for adults with Learning Disabilities. We accommodate up to 8 people with mild to complex Learning Disabilities. We tailor our services to enable people to be as independent as possible. We endeavor to create positive family relationships offering a good circle of support. There is a strong focus on supporting individuals’ lives in a person centred way which includes various activities, shopping, managing own budgets and cooking.
Main Responsibilities
* Develop flexible and realistic support plans and risk assessments with the individuals you support and in collaboration with family members and other professionals, ensuring that the support plans reflect each person's needs, goals and aspirations.
* Review support plans and risk assessments.
* Liaise with health professionals.
* Chair a variety of meetings.
* Complete Mental Capacity Assessments.
* Hold Best Interest meetings.
* Complete DoLS applications.
Ideal Candidate
* Ability to lead a team of Support Workers on a shift, allocate work and delegate tasks.
* Directly line manage a team of support staff, including managing and overseeing the rota, sickness, supervisions and appraisals.
* Manage performance issues, coach and mentor.
* Strong communication skills needed to tailor the message to a variety of audiences.
* Experience in the care sector, preferably a working knowledge of supporting individuals who have learning disabilities, physical disabilities and health issues.
* Open to varied and interesting training and development to develop you as a line manager.
