Location: Birmingham
Essential:
* Band 5 or above.
* Must have a valid driving licence and access to a vehicle.
* Registered nurse with a current NMC pin.
* Relevant Nursing qualifications.
* Hardworking RGN/RN.
* 6 months recent experience working as a Community nurse.
* 2 clinical references that cover 3 years work experience.
* Valid DBS certificate.

Responsibilities:
* Carry out care plans in collaboration with patients, families, and multidisciplinary teams.
* Provide skilled nursing care including wound care, medication administration, and symptom management.
* Co-ordinate care and liaise with other healthcare professionals and community services.
* Maintain up-to-date documentation of assessments, care plans, and interventions.
* Excellent clinical assessment and communication skills.
