Band 6 Occupational Therapist - Community

Pay Rate: £21 - £25 per hour PAYE (DE)

Job Overview:
We are currently seeking an experienced Band 6 Occupational Therapist to join the community team. This is a dynamic and rewarding role requiring the ability to work independently across various community settings. The ideal candidate will have a strong background in complex medical and social needs, along with robust assessment and equipment recommendation skills.
Key Responsibilities:
* Manage and prioritize a caseload of adults with complex medical and social conditions
* Complete comprehensive assessments and deliver personalized treatment plans
* Recommend appropriate equipment, including minor and major adaptations
* Conduct home visits and work within community-based environments
* Maintain accurate documentation and contribute to discharge planning
* Collaborate effectively within the multidisciplinary team to ensure optimal patient outcomes
Key Requirements:
* Degree or Diploma in Occupational Therapy
* HCPC registration as an Occupational Therapist
* Minimum of 3 years' post-qualification experience, preferably within the NHS
* Proven experience managing patients with complex medical and social needs
* Strong background in assessing and recommending equipment and adaptations
* Full UK driving license and access to a vehicle (essential)
