Clinical Team Leader - Community Mental Health

An exciting opportunity has arisen to join Queens Park and Paddington Community Mental Health Hub (CMHH) as a Clinical Team Lead.
We are looking for an experienced Mental Health Nurse to join our dynamic team. The post holder will work with adults in the community with mental health needs and support the team with the day to day running of a busy mental health service.
The role is suitable for a qualified clinician with experience working in the community and for someone seeking to develop their leadership and management skills.
Main duties of the job
The post holder will work as a Team Leader in the Hub with duties including:
* To coordinate referrals and transfers of care
* Be responsible for the supervision of staff & their case load.
* Manage a small caseload
* Support coordination of duty
* Develop and maintain close professional relationships with different teams both within Westminster and external partners
* Supports clinical governance processes, including reviewing Datix incidents and contributing to serious incident investigations under PSIRF.
* Ensure staff practice in a safe, appropriate and professional way with service users and their carers.
* Attend professional practice forums
* Chair clinical and team meetings as appropriate
* Represent the CMHH at various meetings
* Provide senior support to the Team Manager
* To assist the team to be compliant with KPI and monitor electronic records
* Supports staff appraisal, supervision, induction, training and ongoing team development.

Job responsibilities
We're looking for a confident, proactive Clinical Team Lead who thrives on both hands‑on practice and broader service development. You'll carry a small but complex caseload while providing professional and clinical leadership to your multi‑disciplinary team, mentoring staff, coordinating inductions and training and fostering a supportive, learning‑focused culture.
You will drive the design and implementation of community pathways that emphasise recovery, skill‑building and prevention, working collaboratively with the QPP senior leaders to translate strategic priorities into everyday practice.
Collaboration is at the heart of this role: you'll partner with inpatient wards, Home Treatment Teams, primary care, social services and voluntary sector organisations to ensure seamless, person‑centred transitions across the care pathway.
In addition, you'll champion quality improvement through audit, data‑driven reviews and innovation projects, all underpinned by a commitment to compassionate, inclusive care.
Person Specification
EDUCATION & QUALIFICATIONS
* Professional qualification in Mental Health Nursing with current NMC registration or Occupational Therapy.
* Evidence of relevant post‑qualification training aligned to senior clinical practice (e.g. risk assessment and management, supervision, MDT working, service improvement or advanced assessment).
* Leadership and or management qualification or equivalent experience.
* Relevant post‑qualification training such as psychosocial interventions, DBT or CBT skills, motivational interviewing, physical health in SMI or Non‑Medical Prescribing.
Experience
* Experience of working with people experiencing psychosis and complex mental health presentations.
* Experience of leading and contributing to multidisciplinary team decision making and multi‑agency working across statutory and voluntary services.
* Experience of providing clinical supervision, appraisal and professional support to staff.
* Experience of teaching, mentoring or supporting the development of professional staff and students.
* Experience of leading or contributing to service development, quality improvement or pathway development.
* Experience of effective partnership working with internal and external providers to promote socially inclusive outcomes for service users.
* Experience of managing complex caseloads, care planning and clinical risk.
* Experience of managing complex HR issues such as performance management, attendance management or long‑term sickness.
* Previous experience in a leadership role within a mental health service.
* Experience of reviewing incidents via DATIX and or contributing to serious incident investigations under PSIRF.
* Experience of audit activity and action planning to improve quality and safety.
SKILLS, KNOWLEDGE, ABILITIES
* Ability to provide visible clinical leadership and professional guidance within a multidisciplinary team.
* Ability to make timely clinical and operational decisions within agreed governance and escalation frameworks.
* Strong interpersonal and communication skills, including the ability to manage challenging conversations and differing professional perspectives.
* Ability to prioritise workload, delegate effectively and maintain oversight of team activity and service delivery.
* Ability to reflect on practice, use feedback constructively and engage in ongoing professional development.
* Ability to deputise for the Hub Manager when required, providing day‑to‑day operational and clinical leadership.
* Coaching or facilitation skills to support staff development and performance.
* Experience of delivering teaching, training and formal presentations.
