Job Summary
The Care Co-Ordinator will assist clinical personnel in caring for practice patients, including treatment, preventative care, screening, and patient education. The role also involves administrative duties. The Care Co-Ordinator acts as a communication focal point between patients and other clinical and non-clinical members of the practice.
Main Duties
* Work with patients, their families, and carers to improve understanding of health conditions and develop personalized care and support plans.
* Assist patients in managing their needs by answering queries, managing appointments, and providing information to support informed decision-making.
* Help patients access self-management education, peer support, health coaching, and other interventions to enhance health and wellbeing.
* Support patients in accessing training, employment, and benefits, including referrals to social prescribing link workers.
* Coordinate and navigate services for patients and carers, working with social prescribing link workers, health and wellbeing coaches, and other professionals.
* Collaborate with GPs and primary care teams to proactively manage a caseload, including patients with long-term conditions, and refer to other health professionals as needed.

Additional Role Details
This role involves working closely with multidisciplinary teams within the PCN, raising awareness of shared decision-making, acting as a service champion for veterans, cancer, and digital access, and supporting the end-of-life team. The duties may evolve with changing circumstances.

* Maintain confidentiality and adhere to safeguarding protocols.
* Comply with Data Protection Act 2018, Health and Safety regulations, and clinical governance standards.
* Inform management of outside employment or interests that may conflict with NHS duties.
* Follow working time regulations and promote a harassment-free workplace.
* Attend mandatory fire training and adhere to no-smoking policies.
* Subject to enhanced DBS checks and occupational health clearance.
Person Specification
Skills include independent judgment, organizational ability, knowledge of local health pathways, excellent communication, and EMIS readiness. Qualifications required are GCSEs in Maths and English, and NVQ in Health and Social Care or Management. Experience in primary care, working within multidisciplinary teams, and familiarity with QOF are preferred.
