We are seeking a Practice Nurse who has worked within a GP Practice to work in conjunction with our existing nursing team which comprises of Advanced Nurse Practitioners, Practice Nurses and Health Care Assistants (HCAs). Responsibilities will be for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to our patient population. The successful candidate must be well presented and have good IT skills. Previous Primary Care experience essential, EMIS clinical system experience essential. Salary depending on experience.
Main duties of the job
* To plan and deliver high quality integrated nursing care to the practice population
* To promote health and well-being within the practice population
* Running chronic disease management clinics
* To undertake Cervical Cytology Screening
* To implement vaccination programmes for children and adults
* Contraception and family planning
* To work with clinical and non-clinical practice staff, other members of the wider healthcare team and NHSE/ICB in contributing to:
* Deliver objectives set out by the Care Quality Commission (CQC)
* Strongly contribute to the attainment of QOF targets

We are a friendly 8,000+ patient population GP Surgery in Rossington, Doncaster, located in an ex-mining village.
5 Partnership supported by dedicated teams of Advanced Nurse Practitioners, Advanced Clinical Practitioners, Practice Nurses, Nursing Associate, HCAs, as well as friendly management and administration team.
We are also a training practice - hosting student Nurses, Medical Student (3a & 3b), Registrars and Foundation Year 2 Doctors.
Job responsibilities
Providing assessment, screeningand treatment services and health education advice
Assist in the co-ordination of provision of nursing services forthe practice.
Provision of a minor illness clinic, to agreed clinicalprotocols and in participation with the general practitioners as and whenappropriate.
Provide nursing treatments to patients in participation withgeneral practitioners or independently to agreed protocols, to include chronicdisease management.
Provide general and specific health screenings to the practicepatients (within agreed protocols) with referral to general practitioners asnecessary.
Advise patients on general health care and minor ailments withreferral to GPs as necessary.
Supplies and equipment Treatment room and other areas
In participation with the practice manager, ensure themaintenance of equipment and stock levels relating to patient care.
Pathological specimens and investigatory procedures
Following appropriate training, undertake the collection,storage and despatch of pathological specimens (including intravenous bloodsamples in the absence of the Practice Phlebotomist), swabs, smears etc andensure the cleanliness of any equipment so used. Perform any investigatory proceduresrequested by the GPs.
Administrative and professional responsibilities
Participate in the administrative and professionalresponsibilities of the practice team.
Ensure accurate notes of all consultations and treatments arerecorded in the patients notes on the clinical computer system as appropriate.
Ensure accurate completion of all necessary documentationassociated with patient health care and registration with the practice.
Ensure collection and maintenance of statistical informationrequired for regular and ad hoc reports.
Attend and participate in practice meetings as required.
Assist in the formulation of practice philosophy, strategy andpolicy and develop appropriate protocols.
Maintain a notice board in the waiting area designated forpatient health care education.
Student/staff education and training
Participate in the education and training of students of alldisciplines and the induction of all members of practice staff with specialresponsibility for nursing staff.
Maintaineffective liaison with other agencies and staff concerned with patientcare and with all other disciplines within the practice, with appropriateregard to confidentiality.
Maintaincontinued education by attendance at courses and study days as deemeduseful or necessary for professional development.
