Band 5 Nurse – Dorking PCN, Surrey Downs Health & Care
Essential prerequisites:
* Current, valid driving licence – holders must be able to drive in the UK and have access to a vehicle for work purposes.
Key responsibilities
* Contribute to team management and the development of innovative practice.
* Assess care needs and develop, implement and evaluate holistic care packages.
* Treat all patients as individuals, respecting privacy and dignity at all times.
* Implement and evaluate holistic programmes of care.
* Involve, support, inform and educate family and carers.
* Promote health and wellbeing of patients.
* Develop effective communication and liaison systems.
* Maintain clear, comprehensive, signed and contemporaneous records following SDHC procedures.
* Facilitate training and professional updating for the team and students.
* Act as an assessor and mentor to qualified and unqualified staff.
* Maintain standards of evidence‑based nursing care.
* Act in line with NMC guidelines to uphold professional and clinical integrity.
* Support and mentor team members; participate in clinical supervision activities.
* Work independently, maintaining close liaison with senior team members and deputising as required.
* Ensure timely patient care and maintain data entry and other records.
* Co‑operate and partner with all other professionals and agencies involved in patient care.
* Protect vulnerable individuals by acting upon relevant procedures.
