You will join the dedicated team of healthcare professionals at City View Medical Practice, who have an appetite for innovation and have been part of building a successful new primary care model.

You will workas part of our multi-disciplinary team providing personal medical services tothe practice patients. This will include long-term condition management, health screening and promotion,immunisations, treatments and dressings.


Main duties of the job

Our Nursing Team offers a range of clinical services including healthpromotion and long-term condition management, NHS health checks, hypertensionmanagement, contraception and sexual health advice, cervical screening, woundmanagement, travel advice, administration of national childhood and adultvaccinations and phlebotomy. You will work closely with themultidisciplinary team.


About us

City View Medical Practice has a large multi-disciplinary team looking after 15,500 patients. We work across two sites - our main site is at Beeston Hill Health Centre, and our branch surgery is just five minutes' away at Shafton Lane - both of which are modern, purpose-built health centres.

This is a busy practice, where we all work hard to do the best we possibly can for our patients. We like to use everyone's skills and to play to people's strengths. We are a training practice, welcoming GP Registrars, Medical Students, Student Nurses and Apprentices. We encourage every member of the team to extend their knowledge and skills, so that we can better help our patients and improve the flow of work through the practice.

There is a strong focus on support, training,education, assessment and audit, ensuring the team is clinically confident andcompetent to meet simultaneous Practice and NHS England guidance.

Job summary:

The practice nurse will workas part of the primary healthcare team providing personal medical services tothe practice patients. This will include long-term condition management, healthscreening and promotion, immunisations, treatments and dressings.

Essential Qualifications:

* RegisteredGeneral Nurse with evidence of current NMC registration
* Diplomas in chronic disease management e.g. Respiratory, Diabetes, CHD andwillingness to work towards any not yet obtained
* Qualifications/signedcompetencies in the activities listed below and a willingness to worktowards any not yet obtained

Role of the Nursing Team

City View Medical Practice serves the needs of approximately 15,000 registered patients from diverse cultural and ethicalbackgrounds. The team offers a range ofclinical services including health promotion and chronic disease management,NHS health checks, hypertension management, contraception and sexual healthadvice (including device fitting and removal), cervical cytology, woundmanagement, travel advice, administration of national childhood and adultvaccinations, ear care and phlebotomy. We liaise with the multidisciplinaryteam as necessary.

Non-medical prescribers work within the team toprovide holistic episodes of care, so achieving NHS England and ICB mandates tomeet patient need safely, seamlessly, efficiently and effectively by completingclinically robust, evidence-based episodes of care. There is a strong focus onsupport, training, education, assessment and audit, ensuring the team isclinically confident and competent to meet simultaneous Practice and external guidance.

Key roles and functions

* Provide ahigh quality, safe and supportive environment for patients, meeting theiridentified physical and psycho-social needs within the scope of individualaccountable practice.
* Work withinPractice guidelines, policies, protocols and procedures reporting outcomesto appropriate clinical staff so standards of care can be monitored andimproved where necessary. This includes adhering to the following agendas:infection control, equality and diversity, health and safety.
* Maintaineffective communication with patients, carers, relatives and members ofthe multidisciplinary team ensuring information is shared appropriatelyand confidentially so patient needs are met in accordance with NHSdirectives and safeguarding policies.
* Protect andmaintain patient records to the agreed Practice standard and in accordancewith professional record keeping standards (NMC 2018).
* Administermedicines in accordance with a non-medical prescribing framework, patientspecific direction or patient group direction in accordance with thestandards for medicine management (NMC 2018, RPS 2021).
* Organise,manage and prioritise own workload ensuring care of patients is paramount.
* Develop therole by using evidence-based practice and continuously improving ownknowledge as defined by NMC revalidationguidelines.

Job responsibilities:

* Chronic disease management: Respiratory Disease (Asthma/COPD),Diabetes, Coronary Heart Disease. Applicant should hold diploma(s) or bewilling to train
* Assist in/perform routine tasks related to patient care asdirected by senior nursing staff and other clinicians at the practice
* Contraception
* Cytology
* Travel Health
* Removal of Sutures
* ECGs
* Venepuncture
* Routine immunisations / Childhood immunisations
* Assisting GPs with minor surgery and coil fittings
* Chaperoning and assisting patients where appropriate who are beingexamined by another clinician
* Requesting pathology tests, for example urine culture, swabs
* Following agreed clinical protocols with referral to senior nursesor GPs as appropriate
* If appropriate, prescribemedicines in accordance with the RPS Competency Frameworkfor all prescribers (2021) and NMC Code (2018).
* Other tasks will include
* Maintaining and cleaning equipment used by the nurses and other clinicians at the practice
* Maintain general tidiness and cleanliness of nurses' and treatmentrooms, including stocking and rotating items as required
* Maintain stock of waiting room leaflets as instructed by senior practice staff
* Ordering of Health Promotion Literature
* Ordering of vaccinations to maintain stock levels
* Participation in administrative systems in the Practice
* At all times there will be a need to maintain accurate records
* A duty to advise senior nurses of potential problems or errorswithin the range of assigned tasks
* Attending and participating in any Practice meetings when required.
* Undertaking home visits
* Any other delegated duties appropriate to the post

Assignment and review of work

The Nurse is responsibleto the Lead Nurse for clinical guidance and professional management, workreview and formal performance appraisal. Workload is generally autonomouslymanaged within professional and practice guidelines, policies and directives.Communication and liaison with the Lead Nurse is requisite in negotiating andprioritising a realistic workload. The Lead Nurse is contactable throughout theworking day, but in her absence the on- call GP is available to maintain supportand guidance.

* A willingness to undertake additionalcourses, training and education to meet the needs of the position; thisincludes attendance at on and off-site meetings and some study duringnon-working hours.
* A willingness to adapt working practiceto meet changing practice and service development.
* Flexibility of working across sites ifrequired.
* Performance is monitored on an ongoing basis and formally appraisedannually.


Person Specification


Experience

* Working in a multi-disciplinary team;
* Experience of multi-skilled approach to Chronic Disease Management;
* Using electronic healthcare records systems;
* Diplomas/skills in areas key to General Practice, e.g. Phlebotomy, immunisation and vaccination, wound management, health screening, travel health advice
* Have worked with other members of the Primary Care Team (Community Pharmacists, Dieticians etc.);
* Minor illness management;
* Triage in healthcare;
* Participation in research;
* Knowledge and understanding of current national developments and trends in health care


Qualifications

* Registered Nurse (with a current NMC PIN)
* Hold a first level degree in nursing
