The RIACT Team is co-located with health colleagues, promoting joint working for better outcomes. The team includes Social Workers, Community Assessment Officers, Reablement Coordinators, and Occupational Therapists. The primary role involves supporting hospital discharges and community crises.
The team is fast-paced, requiring timely decision-making supported by experienced team members. Assessments include functional, Care Act, safeguarding, Mental Capacity Act, and Continuing Health Care. We focus on strength-based and person-centred practices.
We seek a full-time, experienced or newly qualified Social Worker committed to strength-based practice, capable of gathering relevant information, making sound decisions, and referring appropriately. You should have strong assessment and care management skills and a passion for making a difference.
