Discharge to Assess – Acute Hospital front door OPAL Teams (Queen Elizabeth Hospital and Heartlands Hospital). Working Bank holiday and weekends on a rota system. An exciting opportunity has arisen to join the Acute Hospital Sector Discharge to Assess Teams, based within the Queen Elizabeth and Heartlands Hospital front door OPAL admission avoidance teams.
Permanent. The role involves working across busy Multi-Disciplinary Teams, and may be required to support other teams within the Discharge to Assess Service, as appropriate. The OPAL Team operates seven days a week; you will be expected to work weekends and Bank Holidays on a rota system (one weekend in every four).
You will work as part of the Discharge to Assess Social Work Teams to deliver person-centred practices where citizens are enabled to exercise choice, control, independence and resilience, with a focus on prevention and early intervention. This includes guidance and signposting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014.
You will have demonstrable social work experience, the ability to work positively as part of a Multi-Disciplinary Team, in partnership with other stakeholders, and an interest in and skills to work within the specific Discharge to Assess Multi-Disciplinary Team. You will carry out social work functions within the legislative framework, utilise relevant legislation to complete conversations, provide support and guidance, and produce reports. You will need resilience to deliver under pressure within required timeframes.
