Specialist Physiotherapist – Band 6
Hours: 22.5 hours Per Week
Salary: £22,629-£27,349.60 (£37,715-£45,416 FTE) Depending on Experience
An exciting and rare opportunity for an experienced Physiotherapist to join our specialist Allied Healthcare team, they will be based in the community, to work as a member of multi professional specialist palliative care service to provide a high standard of Physiotherapy assessment, intervention and evaluation for patients and carers referred to the service. This includes an in-reach service to patients admitted to the Inpatient Unit, assisting in discharge planning and therapeutic interventions.
We are seeking an enthusiastic and highly motivated individual to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families.
The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patient’s practical, psychological, spiritual and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, and good communication and team working skills are essential. Foster an environment of partnership working between and within the team and all the stakeholders to ensure positive outcomes for the service users.
Among other duties, the role encompasses equipment provision, moving and handling support, emotional support, providing teaching/education, problem solving and discharge planning.
