Job Summary

Dementia Specialist Community Mental Nurse role within Brighton and Hove Memory Assessment Service. Work as part of a multidisciplinary team, providing specialist cognitive assessment, diagnosis, medication reviews and care planning for people with dementia to support their quality of life and future planning. The role is part‐time permanent (0.8 WTE, 30 hours a week) with flexibility for casual hours as suitable.


Responsibilities

* Conduct comprehensive dementia assessment and diagnostic formulation.
* Provide medication reviews and manage treatment plans in line with NICE guidance.
* Plan and implement individualized care, delivering specialist nursing advice and procedures.
* Explain dementia diagnosis and benefits of service engagement to patients and families.
* Maintain up‑to‑date written and electronic clinical records in accordance with professional standards.
* Identify and manage ongoing risks, reviewing and adjusting risk management plans with patients and carers.
* Adhere to the Ionising Radiation (Medical Exposure) Regulations and other relevant safety standards.
* Attend multi‑disciplinary team meetings to contribute to diagnosis and care planning.
* Engage with patients’ next of kin and carers, supporting their knowledge and involvement in care decisions.
* Supervise and provide clinical guidance to junior staff and trainees.
* Participate in service development, leading on delegated projects and contributing to the strategy of Here and Service.
* Ensure compliance with contractual performance targets and quality standards.
* Maintain professional development, including continuous education and mentorship.


Qualifications (Essential)

* Registered Nurse with current UK registration (mental health, learning disability or adult). Other healthcare professionals with transferable skills (e.g. occupational therapy) may apply.
* Completion of a recognised Mentorship Course or ENB equivalent.
* Evidence of ongoing professional development.
* Right of residency or citizenship in the UK.


Experience & Skills (Essential)

* Significant post‑qualifying experience in a community mental health/dementia setting.
* Experience supervising staff and leading clinical interventions for people with complex health problems.
* Strong assessment, planning and therapeutic communication skills.
* Experience working within multidisciplinary teams and statutory frameworks such as the Care Programme Approach.
* Knowledge of clinical governance, risk management and NICE guidelines.
* IT literacy – confident use of electronic health records, Word, PowerPoint and email.
* Experience in crisis intervention and managing patients with traumatic experiences.
* Excellent negotiation, conflict resolution and relationship building abilities.


Desirables

* Qualified nurse prescriber.
* Experience managing caseloads and conducting local audits/research projects.
* Understanding of clinical governance and Junior staff supervision.


Location & Working Pattern

177 Preston Road, 5th Floor, Brighton, BN1 6AG. Flexible working pattern – part‑time permanent (0.8 WTE) with option for casual hours.


Salary & Benefits

£38,862 to £46,580 a year. Other benefits include NHS Pension Scheme, protected holidays and professional development support.
