The Opportunity
The Young People with Disabilities Service works with young people and adults aged 16-25, supporting their safety, independence, and wellbeing from childhood into adulthood. The team adopts a strengths-focused approach, aligned with ECC’s transformational shift from long-term care support to a model based on prevention, early intervention, enablement, and safeguarding.
This enables young people, their families, and carers to access information and tools, live healthily and independently, and allows the service to respond proactively to increasing demand within financial constraints.
This role collaborates with Qualified Social Workers to deliver tailored, targeted interventions in accordance with care plans to vulnerable adults and young people.
Applicants should have relevant experience in activity-based work with vulnerable adults and/or young people in a social care setting, or be educated to Level 3. Passion for community engagement and direct support is essential.
Accountabilities
* Completing assessments and support planning for adults, and direct work with adults and under-18s, supporting social workers and occupational therapists to implement and monitor care plans.
* Maintaining high standards of professional practice and contributing to continuous service improvement.
* Highlighting safeguarding issues and implementing protective measures for vulnerable adults.
* Building effective relationships with clients, families, and carers, providing practical advice, support, and advocacy to enable safe, healthy, and independent living.
* Collaborating with internal and external partners to deliver joint objectives and improve outcomes.
* Providing information and advice related to accessing third-party services (benefits, immigration, housing).
* Attending meetings, reviews, case conferences, and court hearings as needed to provide insights and evidence.
The Experience You Will Bring
* Experience in social care, with NVQ/BTEC Level 3 or equivalent preferred.
* Commitment to ongoing professional development.
* Proven ability to build relationships with adults, families, and carers in challenging situations.
* Experience in direct activity-based work with adults and/or young people.
* Knowledge of relevant legislation and policies.
* Ability to build partnerships with external agencies.
* First Aid certification and/or Sign Language skills are desirable.
