Responsibilities Conduct comprehensive assessments to determine client needs and strengths. Develop, implement, and evaluate individualized care plans for clients. Collaborate with multidisciplinary teams to coordinate care and resources for clients. Maintain accurate and timely documentation of client interactions and progress notes. Engage in continuous professional development and training to enhance social work skills and knowledge. Requirements Master’s degree in Social Work (MSW) from an accredited institution. Valid state-issued license to practice as a social worker. Minimum of 3 years of relevant experience in social work or related field. Strong understanding of social work principles, ethics, and best practices. Excellent interpersonal and communication skills. Ability to work effectively in a team-oriented environment. Demonstrated ability to handle crisis situations and provide appropriate interventions.
