Job Introduction We have an exciting opportunity for a Bank Registered Nurse Band 5 to join our South Wiltshire Locality Hospital at Home service .Our service is currently going through a positive period of transformation, making this a wonderful time to become part of a supportive, dynamic, and forward-thinking team.The post will be based at either Chippenham, Malmesbury or Savernake Hospital, working Monday - Sunday 8am - 6pm (would consider shorter shifts) As a Bank Band 5 Registered Nurse ,you will be a valued member of the Clinical Team, working across organisational boundaries to bring together the strengths of both acute and community services.Our Step-up team focuses on helping people avoid unnecessary hospital admissions, while our Step-down team supports individuals to leave the acute setting safely and earlier when appropriate. In this role, you will join a group of caring, innovative colleagues who are dedicated to the wellbeing of the local community.Together, you will help ensure that those who use our services receive high-quality, compassionate care that truly makes a difference. Main Responsibilities As a Nurse, you will work with a good degree of independence, carrying out tasks and responsibilities delegated by the lead clinician.You will be an important part of the team, helping to meet the needs of patients in community settings, initially focusing on those referred through the 2-hour Urgent Community Response (UCR), and later supporting the development of broader referral pathways. Working within a multidisciplinary team and across primary, secondary, acute, and social care services in Wiltshire, you will play a key role in coordinating specialist interventions for people who might otherwise require hospital care, or who need timely support to leave emergency departments safely. You will take responsibility for your clinical caseload and help ensure smooth coordination of care throughout each patient’s journey. As part of this role, your responsibilities will include : Clinical practice – Providing evidence-based advanced clinical skills to support appropriate patient care, under the guidance of a registered clinician. This includes assessing patients, interpreting tests within your scope of practice, and planning care and treatment.You will recognise early signs of deterioration, respond promptly, escalate when needed, and document your actions clearly. Management – Organising your time and workload effectively, advocating for patients, managing your caseload day to day, offering advice and treatment recommendations, working closely with clinicians in the Virtual Ward team, and contributing to supervision. Leadership – Serving as a positive role model for colleagues and promoting collaborative working.You will helpchallenge traditional boundaries in a supportive way while upholding high standards of quality, safety, and professional practice. Supporting the learning environment – Demonstrating commitment to your own professional development andsupporting the learning of colleagues and student nurses. The Ideal Candidate Essential Registered nurse Able to work autonomously and as part of a team Experienced in the use of NEWS2 diagnostic tool and when to escalate Contribute to the development of others, supervising and teaching, mentoring, including student Nurses Previous experience in elderly care, frailty, community services or relevant acute care speciality Excellent written and verbal communication, and interpersonal skills Ability to adapt to the demands of a constantly changing environment Good IT skills Work within the NMC code of conduct. Knowledge of community environment Good Communication skills including IT literacy and knowledge of computer systems. Undertake evidence based clinical practice. Experience of caring for people with long term conditions Experience of pressure ulcer prevention Awareness of self-neglect, safeguarding and abusive situations to be the patients advocate. Ability to be an effective team member. Awareness of team dynamics. Work within the boundaries of legislation, policies, procedures, and standards.
