What you will do
* Conduct personalised assessments, support planning, and reviews in line with the Care Act, Mental Capacity Act, and Mental Health Act
* Collaborate with team members including Senior Practitioners, Experienced Social Workers, and Community Care Workers to deliver holistic outcomes
* Manage a varied caseload with complexities such as Physical Disabilities, Mental Health issues, Learning Disabilities, and more
* Engage in Court of Protection matters, S42 Adult Safeguarding Enquiries, and Best Interest Decisions
* Advocate for clients and contribute positively to the team's ethos and outcomes
