Job summary
Partners in Health (part of GDOC Ltd) is looking to recruit a highly motivated Advanced Nurse Practitioner to join our expanding primary care team.
As a valued member of our multidisciplinary workforce, you will deliver high-quality, patient-centred care across our primary care services. This is an exciting opportunity for an autonomous practitioner who is passionate about improving access, enhancing patient outcomes, and working collaboratively within an innovative NHS organisation.
Partners in Health operates as part of GDOC, a well-established not for profit organisation strengthening primary care services and improving healthcare delivery. Main duties of the job
The ANP/AP will work autonomously within their scope of practice to assess, diagnose, treat, and manage patients presenting to the practice. They will provide expert clinical care, work alongside the multidisciplinary team, and contribute to service development to meet the needs of the practice population.

* Undertake comprehensive assessment, diagnosis, and treatment of patients with undifferentiated and undiagnosed problems, both acute and chronic.
* Prescribe and review medication in accordance with national and local prescribing guidelines.
