Our focusis on promoting independence and enabling people to maximisetheir strengths while working collaboratively with stakeholdersto achieve the best possible outcomes. This role plays a vitalpart in delivering high-quality, person-centred support thataligns with the council’s commitment to continuously improvingmodern and competitive services for our community.
In this role, you will work under theguidance of qualified practitioners, holding a caseload andcarrying out assessments to develop tailored support plans. Youwill play an active role in risk management, monitoring outcomes,and ensuring individuals receive the most effective support. Withopportunities for professional development, you will be supportedto grow in your role while upholding national legislation,professional standards, and best practices. This opportunity is within our Support atHome service. Our Service operates between 8am and 8pm, over 7days a week, 365 days a year. Please note: Rota patterns willinclude alternate weekends (as part of a set rota) and BankHolidays.

We are looking for a compassionate andmotivated individual with experience or awareness of social care,healthcare, or the voluntary sector. You should have a Level 3qualification in Care (or be willing to work towards one) andstrong communication skills to collaborate with customers,carers, and stakeholders. A proactive approach to riskmanagement, moving and handling, and health and safety isessential, along with the ability to plan workloads, meetdeadlines, and use IT effectively. Most importantly, you shouldbe committed to promoting independence, influencing positivechange, and ensuring high-quality, person-centred care.
