Job Description
Responsibilities
* Support the practice team to ensure efficient medicine optimisation processes are being followed.
* Follow shared‑care protocols and enhance the quality and safety of prescribing through medicine safety initiatives and project work.
* Respond to medication‑related queries from patients, GPs, allied health professionals and staff, including repeat prescribing, medication review, medicines reconciliation and supply issues.
* Review and act on clinic letters and hospital discharge summaries for medication changes and related monitoring.
* Act on tasks arising from secondary care correspondence.
* Support the practice reception team in routing general prescription requests, enabling clinicians to review complex requests.
* Assist patients in managing their medications, providing information, counselling, and arranging monitored dosage systems, medication synchronisation and electronic repeat dispensing under pharmacy supervision.
* Collaborate with the practice team to highlight patients requiring ongoing monitoring and to promote appointment attendance for long‑term monitoring.
* Act on acute medication requests.
* Maintain accurate and up‑to‑date clinical records.
* Support the practice in delivering PCN DES, QOF, QASI and locally commissioned enhanced services.
* Assist with reviewing and developing practice policies for CQC requirements.
* Implement national prescribing policies and guidance within GP practices, care homes and other primary care settings.
* Present at patient group meetings or events to advise on appropriate use of medicines.
* Participate in training programmes as part of continuous education.
* Ensure adherence to personal revalidation and maintain up‑to‑date competency for the current role.
* Deliver medication‑related training and education sessions for the practice team and other clinical and non‑clinical teams.
* Reports to the Practice Pharmacist Team on a day‑to‑day basis and liaises with all practice staff, attached staff, external agencies and the public.
* Maintain confidentiality at all times.
Person Specification
Qualifications (Essential)
* Good standard of general education and vocational qualifications, NVQ Level 3 Pharmacy Services (or equivalent), Primary Care Pharmacy Education Pathway (PCPEP) or willingness to complete.
* Qualified and registered with the GPhc as a pharmacy technician.
* Evidence of ongoing CPD.
Experience (Essential)
* Ability to deal professionally and efficiently with members of the public, multidisciplinary team and other agencies.
* Passionate about patient/customer care with a flexible approach to cope under pressure.
* Understanding of prescribing and medicines management issues.
* Proven experience in a challenging, constantly changing working environment.
* Strong communication links with staff at all levels and other agencies.
* Ability to organise and prioritise workload and perform within defined timescales.
* Excellent IT skills: Microsoft Word, Excel, Outlook, Teams.
Desirable
* Experience working within the NHS/Public sector.
* Experience working with SystmOne.
