Job Summary
The Pulmonary Hypertension (PH) Service at the Royal Free is part of the national service within the UK. The service is a designated treatment centre for PH by NHS England to diagnose, treat and monitor PH patients. As one of only eight adult centres in the UK, patients with suspected PH are referred to the Trust from a wide geographical area. Once diagnosed, the patients are treated with PH specific therapies which may be given orally, intravenously or inhaled.
The post holder will provide and facilitate the provision of high quality, autonomous, integrated nursing care to PH patients, together with other members of the multidisciplinary team (MDT). They will be responsible for monitoring and assessing patient's disease pathway, initiating treatments and will provide information, education, training and support to this group of patients and their carer(s).
The post holder will provide this service via face to face consultations, telephone clinics and national out-reach programmes. Including the management of the drug monitoring and Intravenous therapy training service for patients at the Royal Free and will be the key support worker to meet the changing needs of this patient group.
The post holder will be expected to understand the necessary administrative systems of the service and to utilise existing documentation to document the outcomes of nursing management and consultations as per policy.
Responsibilities
* To provide an autonomous nursing service using advanced clinical judgment and knowledge of the specialty.
* To facilitate and coordinate the provision of an effective, safe and seamless service for pulmonary hypertension patients and their families and provide information, education and support.
* To act as a resource for patients and the clinical team by the provision of comprehensive advice, information, teaching and training both within the Trust and externally.
* To lead and innovate nursing practice within pulmonary hypertension specialty and add to the body of knowledge within the trust and national networks.
* To contribute to the development of policies, quality standards and evidence-based practice, within the trust and externally.
* Travel to other external Trusts within the Shared care national network throughout the year.
