Kirkintilloch Health and Care Centre

The role:

We are excited to offer an excellent opportunity within NHS Greater Glasgow and Clyde for a Rehabilitation Physiotherapist to join our innovative Rapid Assessment Link service. This rewarding role places you at the heart of community and acute care, helping to prevent hospital admissions and support timely, safe discharges. You will work as part of a highly skilled multidisciplinary team, delivering specialist rehabilitation to patients with complex needs. All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.

Duration, Location, and Working Pattern:

1. You will be based at Kirkintilloch Health and Care Centre, 10 Saramago St, Kirkintilloch, G66 3BF
2. Full Time 36 hours per week, Permanent
3. Parttime hours may be considered

Key responsibilities:

4. Deliver specialist physiotherapy assessment, treatment and evaluation for patients with complex rehabilitation needs
5. Work autonomously, managing a defined caseload while contributing to multidisciplinary care planning and coordination
6. Support early discharge from hospital and prevent avoidable admissions through effective community intervention
7. Act as a key link between acute and community services, ensuring seamless patient care pathways
8. Provide expert advice, education and support to patients, carers and multidisciplinary colleagues
9. Contribute to service development, clinical audit and quality improvement initiatives within the team
10. Supervise and support rehabilitation assistants and contribute to staff development and training

Knowledge, training, qualifications and/or experience required to do the job:

11. Degree or MSc in Physiotherapy with current HCPC registration
12. Significant post-graduate experience, including time at a senior level within relevant clinical areas
13. Proven clinical expertise in rehabilitation, including care of older adults, neurology, orthopaedics and respiratory conditions
14. Ability to work independently and demonstrate advanced clinical reasoning in complex patient care
15. Strong communication, interpersonal and organisational skills with the ability to work across multidisciplinary teams
16. Evidence of ongoing professional development and application of learning to clinical practice
17. Full driving licence and willingness to work across community settings
