Senior Physical Health Check Coordinator
This role is part of the Mental Health and Wellbeing Partnership across Bath and North East Somerset, Swindon and Wiltshire (BSW), which includes Second Step, Alabar, Missing Link and Nilaari. Together, we deliver community-based, trauma-informed support that places people at the heart of mental health and wellbeing services.
Main duties of the job
The Senior Physical Health Check Coordinator supports individuals with severe mental illness, such as psychosis, bipolar disorder and schizophrenia - to access annual NHS health checks and make positive lifestyle changes based on the results.The role is hybrid with an expectation of working the majority of that time out of GP surgeries and a day in the office. It might also require meeting people at or in their homes.

Job responsibilities
Work directly with individuals on the SMI register to help them understand, attend and follow up on their physical health checks.Support clients to access community services such as walking groups, smoking cessation programmes or other wellbeing resources.Collaborate closely with GPs, Primary Care Networks and local voluntary partners to coordinate access to health and wellbeing services.Maintain accurate client records and share insights to improve service delivery.Deliver care in line with trauma-informed principles, valuing lived experience, equality, diversity, and inclusion.
Person Specification
Experience
* A minimum of one year experience working with people with mental health needs, or people with complex needs.
* Proven experience of identifying risk, effective documentation and escalation of risk concerns.
* Experience of working with a number of individuals with competing needs and priorities.
* Proven experience of mental health issues and/or services, either as a user, carer or supporter.
* Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions and offending).
