Background Ensuring hospital bed occupancy remains at safe levels for patients is a critical priority for NHS Lanarkshire and both Lanarkshire Integration Joint Boards. This underpins the wider Scottish Government, Health Board and Health and Social Care Partnership (HSCP) focus on improving flow through health and care services, which is essential to delivering timely, effective care and maintaining system resilience. The Scottish Government has set a national target for NHS Boards to achieve a 5% improvement in unscheduled care performance by March 2026 – and so: Reduce long waits in Emergency Departments (ED). Improve patient flow through hospitals and into community care. Reduce delayed discharges and ensuring timely access to downstream beds. University Hospital Hairmyres (UHH) is significantly challenged in relation to its Unscheduled Care performance; specifically, in relation to long waits in the Emergency Department, access to downstream beds, and effective discharge planning. The reasons for this are varied and complex, and an internal improvement team has been created, working in collaboration with the UHH site leadership team to drive sustained improvement for safe, effective patient care and improved staff experience. Role Profile An opportunity has been created to join the Site Safety Improvement Programme based at University Hospital Hairmyres (UHH) in a Senior Leadership role supporting the development of an Integrated Discharge Hub. In the first instance the focus will be in working collaboratively with the South Lanarkshire University Health and Social Care Partnership (SL HSCP) and UHH teams to establish a sustainable model. The ambition is to create a pan-Lanarkshire Integrated Discharge Hub. The post holder will work across organisational boundaries between the SL HSCP and UHH to provide senior leadership for the development of an effective and sustainable patient discharge model. In the initial stages this senior role will focus on ensuring there are clear roles and responsibilities, robust pathways and processes, enabling tests of change and learning and embedding practice. A key part of this improvement work will be the implementation of robust PDD setting and ensuring effective Multi-Disciplinary Team (MDT) meetings. The senior leader will set a clear direction and ambition for the Integrated Discharge Hub, learning from other national models. Key Objectives Provide Senior Leadership working collaboratively with SL HSCP and UHH teams. Set improvement trajectories ensuring measurements are in place relating to a set of agreed KPIs. Set out a clear plan with associated milestones for the improvement activity required to support the implementation of a site based Integrated Discharge Hub. Evaluate and capture learning from all improvement cycles in order to inform future phases and a Pan-Lanarkshire Integrated Discharge Hub model. A proven track record in the provision of strategic, creative and innovative solutions in meeting organisational requirements, motivating staff and providing seamless patient/client care in new integrated arrangements in conjunction with NHSL; local authorities, public and staff. Ability to provide effective credible communication and an ability to operate effectively under pressure Please note for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence. It would be great if you also have Passion for integration across health and social care pathways Demonstrable understanding of Discharge without Delay principles Contract type Fixed Term or Secondment for 6-months. Full time 37-hours Please note this is a fixed term post for 6-months.
