Primary Care Quality Improvement Facilitator for Cancer
This position includes work primarily in North and North East Lincolnshire.
This post will support primarycare professionals and practice teams across North Lincolnshire & North East Lincolnshire Places.
You will be working inconjunction with the teams at these Places and the Awareness and EarlyDiagnosis Team of Humber and North Yorkshire Cancer Alliance which works acrossthe cancer pathway and spans the whole ICS. As a healthcare system, we areworking towards the NHS Long Term Plan ambition of 75% of cancers diagnosed inthe early stages by 2028 which has been qualified for primary care in a numberof contracts and specifications - for example the Direct Enhanced Service and the Quality and Outcomes Framework.
As one of our team of Primary Care Quality Improvement Facilitators for Cancer you will provide invaluable practical implementation support to PCNs to improve the quality of their services and work towards achieving cancer-related objectives. You will also provide a vital communications channel between primary care and the Alliance work programmes.
Main duties of the job
The PCNs in Humber and North Yorkshire have made varying progress towards meeting the cancer targets - in the context of the added pressure of COVID-19 on primary care and supporting organisations. Humber and North Yorkshire Cancer Alliance have launched a new incentive scheme to support PCNs undertaking actions and improvements across their cancer activities. As one of our team of Primary Care Quality Improvement Facilitators for Cancer you will provide invaluable practical implementation support to PCNs to improve the quality of their services and work towards achieving cancer-related objectives. You will also provide a vital communications channel between primary care and the Alliance work programmes.
The support you will give to PCNs will be allocated across the Facilitators using a needs-based and inequalities-focused approach driven by available data and evidence and taking into account where resources already exist.
You will be a highlymotivated enthusiastic and efficient individual with a passion for improving health outcomes in populations and reducing inequalities. Your background working in the NHS will help you to navigate systems and provide practical solutions. You will have demonstrable experience implementing evidence-based interventions to drive quality improvement in healthcare settings and be prepared to seek out and share best practice.

Job responsibilities
Key Relationships
The post holder will have key relationships with a range of colleagues and organisations including the following teams departments and organisations
* Humber and North Yorkshire Place Cancer Clinical Leads
* Place Cancer Managers
* Humber and North Yorkshire Cancer Alliance Awareness and Early Diagnosis Team
* Communications and Engagement Team
* Broad range of NHS colleagues across the IC S including - but not exclusively - Living With and Beyond Cancer leads Lead Cancer Nurses Primary Care colleagues clinicians and others.
* Other key stakeholders including patient representatives support groups and other external partnerships.
Main Duties and Responsibilities
The following is typical of the level of duties which the job holder could be expected to perform. It is not exhaustive and other duties may be expected to be performed by the post holder from time to time where deemed necessary.
* Build effective relationships with and actively engage Primary Care Networks and local cancer and public health stakeholders to raise awareness of the importance of early cancer diagnosis and influence the content of local delivery plans.
* Effectively manage ongoing relationships with multiple PCNs and practices across your locality to secure improvements in early cancer diagnosis.
* Lead or support workstreams to increase uptake of the three cancer screening programs, improve primary care referral practices and pathways in line with the PCN DES and regional plans - such as the Humber and North Yorkshire Cancer Alliance Cancer Incentive Scheme
* Assist PCNs with the analysis and interpretation of statistics relevant to prevention screening and early diagnosis.
* Share best practice tools and resources to support PCNs to adopt quality improvement interventions that will contribute to achieving earlier cancer diagnosis and better outcomes for patients.
* Support PCNs to develop early cancer diagnosis improvement plans in line with the PCN Cancer Direct Enhancement Scheme Invest and Impact Fund and other relevant contracts.
* Create and facilitate opportunities and mechanisms for PCNs to improve communications and networking between GP practices and PCNs - including joint planning where appropriate.
* Support and - or advise PCNs and GP practices to set up and implement early cancer diagnosis projects focused on screening and improving referral practice.
* Working with clinical leads to support PCNs to keep up to date with changes and the use of clinical referral pathways diagnostic and triage tests - such as Faecal Immunochemical Test
* Develop plan co-ordinate and evaluate learning events for primary care professionals - non-clinical and clinical. To enhance knowledge of and interventions to improve cancer outcomes with a focus on early diagnosis screening and addressing health inequalities
* Design deliver and evaluate bespoke training sessions through a range of methods including face to face remote and local online options at GP practice and PCN level.
* Work in partnership with primary and secondary care partners to support and promote more effective ways of working across the patient pathway.
* Contribute to the delivery of local improvement projects in partnership with NHS partner organisations, in line with Humber and North Yorkshire Cancer Alliance workplan.
* Keep up to date with policy, evidence and NHS/public health cancer strategies, supporting the dissemination to local partner and primary care organisations in the area, as appropriate.
* Work flexibly and collaboratively through the region with the Humber and North Yorkshire Cancer Alliance Place Cancer Managers and other PCN Engagement roles to support early cancer diagnosis projects.
* Provide progress reports against the agreed operational plan and identified performance measures via quarterly reports for Humber and North Yorkshire Alliance in addition to other data intelligence and observations as required.
* Support PCNs and GP Practices to enhance their understanding and implementation of personalised care approaches to improve the quality and uptake of cancer care interventions.

* Degree or equivalent experience
