Care Manager Join Our Clinical Team
Location: Richmond Head Office (2 days per week) & Client Homes (South East) Salary: £28,000 £34,000 per annum (DOE)

About the Role
Paragon Home Healthcare is seeking a dedicated and driven Care Manager to join our Clinical Team. In this role, you will oversee the delivery of high-quality, complex, and specialist homecare for your own caseload of clients.
Youll supervise care teams, deliver and assess training, and ensure care plans reflect the needs of each individual. Supported by our Clinical Director and reporting to the Clinical Manager, youll play a key role in our nurse-led, client-focused services.
This is a varied role, with time split between our Richmond Head Office (ideally 2 days a week), client visits across the South East, and the flexibility of some home working.
Key Responsibilities
Case manage a group of individual clients
Supervise and support care teams, ensuring safe, high-quality service delivery
Deliver and assess staff training in both our training facilities and client homes
Write, implement, and update client care and management plans
Provide direct client care when required
Participate in the on-call rota as part of the wider Clinical Team
Promote client dignity, safety, and independence through compassionate practice
Monitor changes in client health and act appropriately using clinical judgment
Ensure accurate records, risk assessments, and compliance documentation
About You
We are looking for an organised and compassionate leader who shares our commitment to quality care. Youll have:
Health & Social Care Level 4 (or equivalent) and above
Previous experience in field supervision, mentoring, or training
A valid UK driving licence (own car preferred)
Strong organisational and communication skills
Confidence working in clients homes and adapting to non-clinical environments
Proficiency with computer systems and the ability to pick up new technology quickly
A flexible, can-do approach and the ability to work under pressure
