Overview

We are looking for an enthusiastic and committed staff member to join our existing community service. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with the Multi-Disciplinary Team: a spectrum of clinical professionals which includes Nurses, Psychologists, Doctors, Social Workers, Occupational Therapist and support staff. A key focus of the role is undertaking physical health assessments.


Responsibilities

* Work collaboratively with colleagues, service users and their carers to ensure the delivery of focused care and treatment.
* Support individuals who are experiencing a range of mental health conditions, who may be vulnerable, within the service environment.
* Support with physical health assessments; training will be provided.
* Work as part of an established multi-disciplinary team, under the direction of a qualified practitioner, to deliver individualized support in accordance with the agreed care plan.
* Assist in the management and care of Service Users based on individual needs, under care planning and guidance of senior qualified staff.
* Conduct physical health assessments for service users.
* Provide practical support to Service Users in performing day-to-day activities, guidance and education in line with the agreed care plan and for a dedicated period of time.
* Produce accurate and timely documentation; maintain records on all Service Users in line with Trust policy.
* Conduct risk assessments of individuals and potential hazards in the Service Users environment and report concerns to senior staff.
* Attend the weekly Multi-Disciplinary Team meeting.
* Support the service and Associate Practitioner by conducting physical health assessments for service users.

About the Organization and Induction

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations to join up health and care services, improve population health and reduce health inequalities. The HCA Academy is an ICS initiative to support the incoming Health Care Assistant workforce, with a five-day face-to-face induction at the MSE ICS Education Hub, Brentwood Community Hospital. Your start date will align with one of the HCA Academy induction dates, discussed during recruitment.

The HCA Academy Induction programme trains and supports new staff through the early stages of employment at EPUT and provides opportunities to enhance skills, knowledge and experience. Further information will be provided post interview. This will be followed by an additional five-day EPUT corporate/mandatory training welcome (delivery may be face to face or virtual).
