Mid Dorset Primary Care Network is seeking a dedicated anddriven Service Coordinator for a maximum of 20 hours per week. Successful candidates will beresponsible for managing the administrative aspects of supporting our long term conditions clinical team for respiratory, cancer, mental health. There will also be a level of support for the central management team.
This is a part time position that would suit the hours between 10a.m to 2p.m Monday to Friday, although we would be flexible for the right candidate.
This is currently a fixed term for one year contract.
The successful candidate would primarily work within our Poundbury site but may need to work also from our head office in Puddletown, and other Mid Dorset locations, therefore own transport is essential.
You will need to support the team and patients so excellent communications skills are a priority. So if you have excellent team working, organisationalskills and the ability to interact with a range of people then we would like tohear from you.

Main duties of the job
Manage and coordinate support for relevant healthcareprojects.
Assist specialist clinical leads in the delivery ofday to day and project tasks.
Assisting with audit and data gathering.
Support the development of early detection of patients witha long-term condition.
To assist and support GP Practices within Mid Dorset asguided by Project leads and PCN Managers.
To support the management teams in coordinating meetings, writing reports/minutes.

Job responsibilities
To assist the Primary Care Network team to deliverimprovements to the services we provide to Mid Dorset Patients
To support the participation of national programmes for long-termconditions
Ensure robust and supportive referrals in place forpatients where services dictate.
Form productive and supportive relationships with practicestaff in support of long-term conditions where possible and relevant.
Contribute to the agenda by collaborative working withvarying levels of communication.
Support audit/referralsand case reviews.
Where guided, develop safety netting advice where patientsare referred for long-term conditions.
Support the PCN in its delivery of peer-to-peer learningevents.
Work alongside other organisations, delivering long-termcondition agenda.
Be the nominated contact for respiratory, Mental Health, Cancer
Maintain accurate records and documentation.
Ensure compliance with data protection and confidentialityregulations.
Generate reports and summaries when requested or to supportinitiatives.
Provide educational material to patients when required byemail, post, text message.
Assist in the organisation of events and meetings, minutetaking and report writing.
Maintain robust and responsive lines of communication.
Respond to queries and demands of the role quickly andefficiently.

* Clinical or non-clinical NHS experience.
