Queens Park Medical Centre is looking for a PharmacyTechnician to join our friendly and enthusiastic multi-disciplinary team.

The post holder will work directly for thepractice to support the team and ensure the delivery of safe, effective andefficient systems for repeat prescribing, medicines optimisation, reducingmedicines waste and maximising patient outcomes.


Main duties of the job

We recognise the value and better need to utilise the roleof pharmacy technicians within primary care to help patients stay safe and wellout of hospital and are therefore looking to expand on our existing pharmacyteam.

The purpose of this role is to lead improvements to maximisesafe, cost effective best practice in prescribing to improve the quality ofpatient care. The post holder will play a pivotal role in helping patients toget the best from their medicines through effective medicines reconciliationafter discharge and on other transfers of care, medication use reviews,identification of patients suitable for electronic repeat dispensing, promotionof online ordering, providing assurance of monitoring of high risk drugs, andhandling of prescription queries within their areas of competence.

The role will also include supporting and eventually leadingthe training and development of practice based prescribing clerks andadministrators to improve co-ordination and effective pharmacy administrationwithin the practice, especially with relation to repeat prescribing systems andprocesses.

The post-holder will also assist the general practice teamin achieving national requirements, NICE implementation, and utilisation ofmedicines optimisation initiatives.


About us

We are proud to offer a friendly and welcoming environmentproviding excellent healthcare to the residents of Stockton-on-Tees.

We are a large practice with 18,700 patients. We have 10 GPs, 4 Nurse Practitioners, 5practice nurses, 3 HCAs, a Practice Pharmacist, a team of prescribing clerks,and a superb group of highly committed and enthusiastic administrative andreception staff. We also work closelywith our PCN team which includes Social Prescribing Link Workers (some withspecialist skills in cancer care and learning disabilities), First ContactPractitioners, Health and Wellbeing Coaches, Pharmacists and PharmacyTechnicians.

We are also a training practice and our GPs are experiencedin supporting the PCN pharmacy team and our own practice pharmacist. We would be keen tohear your ideas and support your individual progression within the role.

The work is fast paced and challenging but rewarding and youwill be part of a brilliant team.


Job responsibilities

The Pharmacy Technician will work alongside our prescribingclerks and under the supervision of our clinical pharmacist to:

·Be responsible for the organisation, planningand of own workload to meet set deadlines

·Use and maintain information reporting systemsand databases relevant to the position including incident reporting, medicinesoptimisation, prescribing data, cost savings, and patient outcomes

·Take a proactive approach towards the safehandling of prescriptions, including assessment of risk and makingrecommendations for improvement

·To respond to medication queries from patientsand staff in a professional manner and within practice protocol, referring tothe appropriate clinician when required

·Liaise with other pharmacy stakeholders e.g.Medicines Optimisation/Community Pharmacy/Hospital Pharmacy/PCN Pharmacy teams

·Support and eventually deliver training anddevelopment to practice based prescribing clerks and administrators to supportimproved co-ordination and effective pharmacy administration

·Undertake medicines reconciliation reviewspost-discharge and on other transfers of care, within the remit of practiceprotocols and growing individual competency

·Run medication safety audits and highlightpatients that require clinical review e.g. in response to MHRA / nationalpatient safety alerts, the local incentive scheme, quality and outcomesframework (QOF) or local updates

·Help review and contribute to the practice'srepeat prescribing and high-risk drug monitoring protocols

·Undertake medication use reviews and support thepharmacist by identifying patients who need further clinical medication reviewand assisting where able, e.g. through the provision of patient safetyinformation leaflets, joining on community visits etc.

·Participate in practice MDT meetings, patientparticipation groups, and other meetings to promote issues relevant toprescribing and medicines optimisation

The postholder will also:

·Ensure the principles of openness, transparencyand candour are observed and upheld in all working practices.

·Have or acquire through training provided by theorganisation, the appropriate level of safeguarding and knowledge, skills andpractice required for the post and be aware of and comply with theorganisation’s safeguarding protection policies and procedures.

·Ensure that any infection prevention and controlissues are reported to the line manager/Infection Prevention and Control


Person Specification


Qualifications

* Professional registration with the General Pharmaceutical Council as a pharmacy technician
* NVQ level 3 Diploma in the Principles and Practice for Pharmacy Technicians or similar
* Enrolled in, be qualified from or willing to embark on an approved training pathway, for example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH) by CPPE


Experience

* Evidence of ability to work proactively and independently
* Computer literate with an ability to use Microsoft Office Packages
* Knowledge of SystmOne clinical system
* Experience of working in a general practice setting
* Knowledge of the principles of medicines optimisation
* An appreciation of the NHS agenda and the Quality and Outcomes Framework (QOF)
* Good clinical pharmacy knowledge including terminology
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing
* Knowledge and understanding of pharmacy law and ethics and current legislation


Skills and Qualities

* Ability to work in a busy and sometimes changing environment with demonstratable time keeping and prioritisation skills
* Able to prioritise own workload, under own initiative within the boundaries of the role
* Awareness of own personal and professional limitations, and willing to seek advice where necessary
* Attention to detail, able to work accurately, identifying errors quickly and easily
* Committed to own CPD, with support from employer
* Understanding of data protection and confidentiality issues
* Works effectively independently and as a member of a team
* Written and verbal communication skills with the ability to adjust communication style and content to suit the audience
