Join our friendly and supportive Dorking team and make a real difference to patients in your local community. We’re looking for a committed Community Nurse to help us deliver outstanding, patient-centred care. Whether you’re newly qualified or an experienced nurse, we’d love to hear from you.
In this role, you’ll provide high-quality nursing care to patients in their own homes, care homes, and clinics. You’ll see a wide variety of conditions and needs, from wound care and palliative support to long-term condition management and rehabilitation, working closely with a skilled and multi-disciplinary team.
Surrey Downs Health & Care provides care closer to home through our Community Nursing Teams, Primary Care Networks, Community Hospitals, and Specialist Services, working as part of an innovative partnership of local NHS organisations.
Responsibilities
* Provide high-quality nursing care to patients in homes, care homes, and clinics.
* Assess care needs and develop, implement, and evaluate holistic care packages.
* Contribute to team management and the development of innovative practice.
* Support students and participate in supervision and ongoing training.
* Support professional growth and operate within a flexible working model across sites.
Our Community Nurses deliver person-centred care that crosses traditional boundaries by working across GP practices, patients homes, care homes and clinics to ensure local people receive the right care, in the right place, at the right time.
