This post offers regular supervision, in‑service training sessions and the opportunity to develop clinical competencies as part of your appraisal. * Identify the most appropriate form of ongoing assessment including functional assessments, seating, upper‑limb and need for splinting along with standardised cognitive and mood screens.
* Set patient‑centred goals, devise and regularly review treatment plans to achieve these.
* Work closely with the MDT to identify rehabilitation potential and the most appropriate setting for this to occur (e.g. stroke rehab unit / own home).
* Communicate effectively with patient, family and wider organisations to support patient recovery and coordinate a safe transfer of care / discharge.
* Effectively manage own caseload, prioritise and delegate to Generic Therapy Assistants.
* Identify all necessary equipment to facilitate discharge and complete timely referrals to equipment suppliers.
* Complete clear and concise referrals to other services and coordinate a timely discharge to ensure maintenance of patient flow.
* Ensure compliance with mandatory training and ensure all trust policies are followed.
