We are assisting a Local Authority in the appointment of an experienced Health and Social Care Officer to join their Adults Social Care Team in Nottingham. As a Health and Social Care Officer provide advice, triage and coordinate information to enable decision making and risk management on behalf of citizens requesting / requiring access to social care and health services, and to navigate citizens through to the right services based on an assessment of their needs. Key Responsibilities: - Coordinate telephone calls, enquiries and referrals from citizens, vulnerable adults, families and carers and to manage these in an effective, timely and customer focused way. Deliver an agreed level of customer service that reflects the vision, values, aims and objectives of Nottingham City Council and partners. Manage specialist or escalated calls from Health and Social Care Support Coordinators, Health and Social Care Support Advisors or other colleagues. To have an understanding and knowledge of the variety of relevant services and their referral criteria available within social care and health to enable appropriate advice and guidance is given and to ensure this information is kept up to date.
