Ensure all recommendations from the MDT and its outcomes are recorded on the Evolve system immediately after the MDT and are actioned accordingly.
Produce the list of all patients to be discussed at the Benign MDTs, distribute to MDT members prior to the meeting, and circulate a list of all patients discussed afterward, indicating the main conclusions from the meeting.
Prepare the weekly MDT by completing proformas, including all necessary test results, to ensure patients are discussed at the weekly MDT.
Coordinate with other MDT coordinators and across the Trust and other hospitals to monitor all elements of the patient pathway. Highlight patient target dates to ensure investigations are carried out in a timely manner.
Work closely with clinicians, specialist nurses, secretaries, and all MDT members across Colorectal to ensure symptoms are diagnosed and actively managed through patient pathways.
Undertake any other reasonable duties as directed by the Colorectal clinical and non-clinical teams.
Maintain confidentiality regarding patient matters and do not disclose such information to unauthorized persons.
Utilize new working systems within the Clinical Office, such as Dictate IT, E-RS, Cerner, Evolve, CRIS, ICE, and store relevant information electronically on shared drives to promote efficiency.
Main duties include being fully familiar with patient pathways, ensuring equitable treatment, and meeting targets for appointments, tests, and treatments within relevant timescales.
Monitor the coordination of patient pathways across Trusts and highlight patient target dates for investigations.
Work closely with the MDT team to ensure timely diagnosis and management of symptoms.
Prepare weekly MDTs by completing necessary documentation and test results.
Record MDT recommendations on Evolve immediately and ensure they are actioned.
Create and distribute patient lists for MDT discussions, noting main conclusions.
