We are advertising for a full time Community Staff Nurse to work within the Integrated Community Team based in Harlow and covering its geography supporting the Care Closer to Home model.
The post holder will be working as part of the Integrated Neighbourhood Teams supporting the Harlow Integrated Neighbourhood Team with a focus on prevention of admissions and facilitating hospital discharges.
The post holder will be responsible for assessment of care needs, development, implementation and evaluation of programmes of care for patients within the Team Leader's sphere of care.
The post holder works without direct supervision and is responsible for nursing care for the geographical area including weekends and evenings.
Main duties of the job
* Takes responsibility for a patient or a defined group of patients within their own homes or elsewhere as defined by the Senior Nurse.
* Undertake comprehensive nursing assessments to identify the patient's physical and environmental needs.
* Plans, evaluates and prioritises interventions and care for patients within his/her sphere of responsibility.
* Leads the clinical nursing management of the caseload's most dependent patients. Determines, in liaison with the Senior Nurse the frequency, type and monitors the efficacy of patient care offered.
* Regularly review caseload patient care, informing the Senior Nurse of any changes made.
* Establish priorities in his/her workload - organising his/her workload to make effective use of time.
* To act up in the Senior Nurse's absence.
* Responsible for the delivery of nursing care for a geographical area including weekends and evenings.
* Liaise with other agencies and members of Primary Health Care Team and secondary care.

Job responsibilities
To care for patient within the community at patients home. Provide care for those at home requiring health interventions such as:
* Wound care
* Palliative care and End of life
* Medication administration
* Diabetes management
It is imperative that the applicant works professionally within their clinical scope adhering to trust policies and within NMC standards of care.

Skills
* Car owner/able to use car for business purposes
* Valid driving license
Qualifications
* Registered General Nurse
Other Skills

* Continuing Healthcare knowledge
