The role will include tasks such as:
1. Helping people to leave hospital promptly and transition back to independent living at home.
2. Visiting Service Users at home to provide care and support as per the Care Plan.
3. Providing personal care, administering medication, monitoring vital signs, assisting with exercises, and domestic tasks if needed.
4. Liaising with medical professionals like district nurses and pharmacies.
The goal is to rehabilitate and maximize the potential of Service Users, supporting activities that promote independence and monitor well-being.
Working hours are between 7am-10pm with shift patterns of 7am-2pm and 4pm-10pm, including alternate weekends.
We offer competitive pay rates of £13.43 per hour on weekdays and £15.50 per hour on weekends.
* The hourly rate applies if you hold an NVQ Level 2 or above in Health and Social Care.
