Responsibilities
* The Clinical Team Leader is a clinical expert who will lead the Primary Care nursing team to deliver a targeted and appropriate service in accordance with evidence-based practice and local policies and procedures.
* Deliver care in a range of nurse-led clinics utilising the Quality Outcome Framework and develop services such as long-term conditions management, nurse-led minor ailments clinics and immunisation programmes.
* Demonstrate a professional manner, being tolerant, courteous and calm; deal with patients professionally and manage potentially volatile situations to avoid escalation.
* Promote equal opportunities in accordance with local and national policies.
* Promote high standards of service within the Primary Care nursing team, manage performance, and contribute to a culture of professionalism.
* Work with the Primary Care Lead and other Healthcare managers to develop and implement an effective Primary Care Strategy.
* Support delivery of a responsive, dependable and high-quality primary care and mental health service within Health & Justice.
* Engage in continuous professional development through the clinical leadership career pathway and relevant training opportunities.

Detailed job description and main responsibilities
Clinical
* Plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models.
* Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for patients with physical health needs, including those with mental health and/or substance misuse needs.
* Safeguard vulnerable patients in line with Trust safeguarding policies.
* Provide health education, advice and information to patients and develop health promotion programmes in partnership with the prison.
* Provide specialist clinical advice, training and support to nursing colleagues on patient management.
* Apply local risk assessment and management protocols.
* Refer to other services for long-term or complex care management (e.g., detox, counselling, psychology).
* Advise primary care nursing staff and GPs on medication issues; promote adherence to local policies and professional standards.
* Be aware of security implications of medicines within Health & Justice Services.
* Monitor health status of patients and address issues; coordinate multidisciplinary care and integrated treatment programs.
* Develop clinical services with primary care staff to deliver nurse-led clinics (minor ailments, chronic disease clinics, health promotion).
* Use de-escalation techniques for aggressive behaviours and follow appropriate procedures.
* Collaborate with prison/education staff to provide integrated care and work within the team’s competency; communicate training needs to the team manager.
* Lead local audits and participate in Trust audits to improve quality of care.

Education & Qualification
Essential criteria
* NMC Registered Adult Nurse
* Education to degree level, or appropriate experience
Desirable criteria
* ENB998/SLICE or Mentorship qualification
