Overview

We are looking for a passionate Advanced Clinical Practitioner with surgical skills who would be keen to develop further and to have opportunities to develop their clinical assessment skills. The post holder will be based at West Suffolk Foundation Trust (WSFT) within the Dermatology Department. The post holder will use a high degree of autonomy and complex decision making alongside demonstrating the four pillars of practice: Clinical Practice, Leadership and Management, Education and Research.

The ACP is accountable for independent clinical assessment, examination, diagnosis and treatment within an agreed scope of practice in dermatology. The post holder will also be responsible for appropriately referring patients to relevant specialties for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge. Advanced Dermatology skin surgery, rapid access clinics and skin analytic clinics, and teaching nursing staff basic skin surgery are part of the role.

The ACP will work within WSFT’s Dermatology department under Medicine and will provide healthcare that enables innovative solutions to enhance experience and improve outcomes within the speciality.


Responsibilities

* Communicate effectively with patients, carers, colleagues, and others using appropriate communication styles. Ensure timely, accurate and complete documentation reflective of Advanced Clinical Practice, consistent with legislation and local/national requirements.
* Discuss and agree assessment outcomes with patients, carers, and other health care professionals to enable informed decisions regarding treatment.
* Communicate complex information to patients, carers and families during consent and explain treatment options. Provide empathy and reassurance and support patients and families through difficult information.
* Ensure informed consent, anticipate barriers to communication, and keep patients informed about changes in treatment plans.
* Deescalate potential complaints and hostile situations with staff, patients and carers using negotiation and interprofessional skills.
* Provide high-quality written and verbal information to the wider MDT for effective patient management.
* Promote health, assess patients for risk factors and early signs of illness; maintain effective multidisciplinary team communication for changes in patient conditions or plans.
* Develop partnerships with patients, families and carers to support wellbeing; establish links outside the organisation to share good practice and promote service development.
* Establish and maintain regional and national networks of contacts relevant to the role; promote the service through presentations, study days, conferences and papers.
* Be a core member of the multidisciplinary team providing specialist knowledge and promote collaborative working relationships across the team.
* Plan and organise work efficiently with excellent time management and personal organisation; contribute to teaching programmes and staff development; develop clinical pathways in collaboration with the MDT.
* Deliver education and training to trainee ACPs, nurses, medical and other disciplines to ensure care is based on research and best practice.
* Work in line with WSFT's Advanced Practice ethos and demonstrate the Trust's values and behaviours to the highest standard.


Clinical Duties

* Be a registered healthcare professional with MSc in Advanced Clinical Practice or recognized equivalent; independent non-medical prescribing eligibility if appropriate.
* Practise at an advanced level underpinned by four pillars: Clinical Practice, Leadership and Management, Education and Research.
* Demonstrate in-depth knowledge and competence in dermatology surgery and lesion recognition with advanced clinical assessment, critical thinking and evidence-based management.
* Provide expert independent clinical care coordinating a multi-professional seamless service; use a diverse knowledge base for therapeutic decisions, including prescribing per Royal Pharmaceutical Competency Framework and local policies.
* Plan and manage complete episodes of care, delegate appropriately, and support patients and clients to optimise outcomes and resource use.
* Promote clinical excellence across the ward/area by advising and supporting nursing staff and other healthcare professionals.
* Autonomously assess, plan and implement management for undifferentiated presentations within the defined scope of practice; obtain patient history and perform physical examinations to inform diagnoses and treatment.
* Conduct clinical procedures, including invasive testing and high-skill treatments; continually evaluate patients and adjust treatment plans in consultation with the MDT.
* Diagnose using multiple data sources and present information to specialty/senior medical staff for advice when scope is exceeded.
* Prescribe and review medications as an independent prescriber, within evidence-based practice and local protocols; monitor effectiveness of interventions and integrate pharmacological and non-pharmacological treatments.
* Prioritise health problems, intervene in complex/urgent situations, and support patients and families through illness and treatment.
* Respect dignity, privacy, and cultural beliefs; manage clinical risk and adhere to professional codes of conduct.
* Follow national guidance and local protocols to analyse clinical situations and formulate management plans; manage own workload in line with service needs.
