An exciting opportunity is available within Adult Care, Housing and Public Health for a qualified Social Worker in the IDT (Integrated Discharge Team)- Transfer of Care Hub

We cover a range of areas such as intermediate care bed bases and supporting discharges from out of area hospitals. We work collaboratively with health partners in the Transfer of Care Hub and complete Care Act assessments for people accessing Integrated Rapid Response.
This is an exciting time to join the team with the development of our Transfer of Care Hub, you will be an integral part of MDT working for hospital discharges and progression of D2A pathways.
Successful candidates will join a supportive team which is comprised of health and social work professionals based at Woodside. As part of this role, you will be expected to support others in the team, including Assessment and Review Coordinators to develop their skills and experience.
About You
We are looking to recruit exceptional Social Workers with the right values, skills, knowledge and experience to contribute to further enhancing and embedding strengths-based practice; focusing on people and the outcomes they wish to achieve.
As our ideal candidate you will:
* Have knowledge and experience of applying statutory legal framework and policies including the Care Act 2014, the Mental Capacity Act 2005 and Making Safeguarding Personal.
* Be confident in working at pace in challenging situations, to make sound decisions quickly to support those in crisis and to ensure people are seen and supported in a timely manner.
* Have experience of working with health colleagues, voluntary sector organisations and other partner agencies to help people find solutions to maximise their independence and resilience and enable people to take positive risks and achieve their goals.
* You will demonstrate initiative and be able respond independently to unexpected situations.
