We are seeking a skilled occupational therapist to join the CLCH ICSS multidisciplinary team, providing a comprehensive service to residents across West Hertfordshire, both in their homes and in community settings. The ICSS includes the Early Supported Discharge Team (ESD), Neuro Enhanced Transition Team (NETT) and the Community Stroke Team.

As an autonomous practitioner, you will work within professional standards and clinical guidelines, promoting best practice while managing your own caseload. This includes assessing, treating and documenting care for patients/clients in line with service protocols.

You will build and maintain strong professional relationships with referrers and partner services, including GPs, acute providers, Bedded Services, District Nurses, Wheelchair Services and other CLCH teams, to ensure timely and appropriate referrals. You will also have a clear understanding of the occupational therapy role within a multidisciplinary team.

This role includes supervising rehabilitation assistants and supporting occupational therapy students, contributing to their professional growth. Work is primarily conducted in patients’ homes, but may also take place in clinics, residential and continuing care homes and community centres.

You will be supported in your personal and professional development, with opportunities to participate in Continuing Professional Development (CPD), audits and service‑led research initiatives.

Conduct multidisciplinaryneurological assessmentsto identify clients’ needs and collaborate with the MDT on joint goals. Clients may present with a variety of complex neurological conditions.

Perform specialist occupational therapy assessments, includingfunctional, cognitive, visual, vocational, seating and upper limb assessments, and identify priorities for patient‑centred interventions.

Work collaboratively with other healthcare professionals to ensureholistic care planning and delivery.

Supervise and mentorrehabilitation assistants and occupational therapy students, supporting professional development within the team.

Maintain accurate, timely and professional documentation for all assessments, interventions and communications.

Actively participate in CPD, service audits and research projects to support continuous improvement and evidence‑based practice.
