Job Summary
We are looking to recruit experienced, caring, compassionate and committed ACPs who, acting within their professional boundaries, will provide care for patients presenting with undifferentiated, undiagnosed problems from initial history taking, clinical assessment, diagnosis, requesting of diagnostics and creating a management plan. They will demonstrate safe, competent clinical decision-making and expert care.

They will demonstrate safe, competent clinical decision-making and expert care.
Positions are available for experienced ACPS in our UTC based at St Peters Hospital and/or the 3 GP acute illness clinics in Chertsey, Ashford and Woking.
The post holder will communicate and work collaboratively with all members of the multi-professional team to meet the needs of patients, and work to improve the quality and safety of care offered to the registered patients in the locality of North West Surrey.
Main duties of the job
Various Shift Patterns Available:
* Acute Illness Clinic is Monday to Friday
* UTC covering 7 days a week.
The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the multi-disciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
* You will assess, diagnose, plan and implement treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
* You will utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
* You will recognise and work within own competence and professional code of conduct as regulated by the appropriate governing body.
* You will take responsibility for own learning and performance, participating in clinical supervision and in yearly appraisal, personal development plans and revalidation.
* You will manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
* You will attend mandatory training courses and take responsibility for own professional development, undertaking learning as appropriate.

Support and manage health needs of women presenting for family planning, or sexual health consultation

Communicate with and support patients who are receiving bad news
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
Delivering a Quality Service
Recognise and work within own competence and professional code of conduct as regulated by the appropriate governing body
Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures
Deliver care according to NSF, NICE guidelines, and evidence-based care
Work with others on improving the quality of health care by applying NICS policies, standards and guidance and national policies and initiatives as appropriate.
Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation including clinical audits, supporting the implementation of improvements where required
Evaluate patients response to health care provision and the effectiveness of care
Support investigation reviews and management, and identify learning from patient complaints, clinical incidents, and near-miss events to be shared across the service and wider organisation.
Participate in the ongoing maintenance of quality governance systems and processes across the organisation and its activities
Work with Lead Nurse to ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Provide guidance and support as appropriate.

* Qualified Nurse/ Paramedic with Masters Level Advanced Clinical Practice or equivalent via Credential/portfolio route.
