Salary: Up to £29,000 per year (depending on experience) + performance bonus

We are partnering with a reputable community care provider in Manchester North & Salford that is looking to recruit an experienced Care Coordinator to join their growing domiciliary care team.
This is an excellent opportunity for someone with experience in home care coordination who enjoys balancing operational organisation with client-focused care. The successful candidate will play a key role in ensuring care services are delivered efficiently, safely, and to a high standard.

About the Role
This role combines office-based coordination, field-based client visits, and participation in a shared on-call rota to support the out-of-hours running of the service.
Office-Based Responsibilities
Create and manage staff rotas and travel schedules to ensure continuity of care
Maintain accurate records in line with CQC standards and GDPR requirements
Liaise with service users, families, care staff, and external professionals
Update and review care plans and risk assessments
Record and manage incidents, complaints, and safeguarding concerns appropriately
Support the day-to-day coordination and smooth operation of the care service
Field-Based Responsibilities
Conduct initial client assessments and periodic care reviews in clients’ homes
Monitor service delivery to ensure compliance with policies, procedures, and regulatory standards
Build strong relationships with clients and their families
Support care staff where required and ensure health & safety compliance
On-Call Duties
Participate in a shared on-call rota with the management team
Respond to out-of-hours queries, emergencies, or staff issues
Support continuity of care and ensure service users receive appropriate assistance when required
Requirements
Full UK driving licence and access to a reliable vehicle
At least 1 year of experience in domiciliary/home care coordination
Strong communication and interpersonal skills
Excellent organisation, time management, and attention to detail
Experience with rota planning and travel coordination
Confidence using care management systems (e.g. PASSsystem or similar)
NVQ Level 3 in Health & Social Care (or working towards it)
Ability to support, motivate, and guide care staff effectively
