The Community Mental Health Nurse/Occupational Therapist is a key clinical leader within the multidisciplinary care team. The role involves assessing, planning, coordinating, delivering, and evaluating care, communicating findings, influencing change, and promoting health, best practices, patient safety, and experience.
The post holder will be professionally accountable to the Executive Director of Nursing and operationally accountable to the Service Manager/Matron. Candidates must be able to work competently in a challenging environment, adapt to changing care demands, and demonstrate high levels of care and empathy. The role requires a full, valid UK driving licence and access to a vehicle for business purposes, unless exempted by disability as per the Equality Act 2010.
Key Responsibilities:
1. Conduct comprehensive assessments considering mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service users and others.
2. Collaborate with multidisciplinary teams, service users, and carers to develop holistic, person-centred care plans.
3. Deliver and evaluate safe, person-centred care supporting recovery.
4. Support best practice in line with NICE guidance as a clinical leader.
5. Facilitate and review quality improvement initiatives within the care setting.
6. Support patients in functioning optimally in their environment, encouraging self-care and providing physical care as needed.
7. Maintain a safe environment, adhering to infection control protocols and reporting hazards promptly.
8. Escort and transport patients as part of their care plan.
9. Drive own vehicle as required and based on individual capabilities.
10. Serve drinks and promote healthy choices according to individual care plans.
11. Work a full range of shifts as required, possibly at different trust sites.
12. Physically capable of performing all duties, following physical interventions training.
13. Review, evaluate, and share service information to ensure value-for-money and continuous improvement.
