About the Role
We are seeking part-time Band 3 Community Support Workers to support holistic and personal care for individuals at the end of their lives. This role involves providing domiciliary care to a diverse patient group, offering short-term services while longer-term care arrangements are being made.
Support will be provided in patients' homes to prevent unnecessary hospital admissions, facilitate earlier discharges, and support in-reaching to hospital wards. We prioritize continuous professional development and will provide comprehensive training and ongoing support to help you develop the skills needed to work independently and confidently.
Key Responsibilities
1. Work with District and Community nurses to deliver wellbeing care plans.
2. Support patients receiving palliative care at home, including personal care, mobility assistance, wound care, medication prompting, and helping with eating and drinking.
3. Ensure a clean, safe environment and promote independence, health, and wellbeing.
4. Provide emotional support and mental health signposting, including linking patients to appropriate services.
5. Participate in assessing patient needs, preventing avoidable hospital admissions, and supporting earlier discharges from hospital.
6. Report and communicate effectively with the multidisciplinary team.
Additional Information
This position requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants subscribed to the DBS update service can provide a valid certificate instead of a new check.
