An exciting opportunity has arisen to join Wolds Community Wellbeing team as a Care Coordinator.
Under current legislation, primarily the Care Act, Mental Capacity Act 2005, and the Human Rights Act, you will be responsible for completing strength-based assessments, care and support planning, and undertaking reviews with service users in their own homes. A key aspect of this role is the promotion of independence, wellbeing, and choice, in line with strength-based assessment principles.
The role involves working with people over the age of 18, their families, and carers, with a focus on supporting individuals to remain in their own homes.
Responsibilities include visiting care homes and hospital settings. We are seeking an individual with experience working with adults, well-developed communication skills, and confidence to collaborate with a range of professionals to ensure the eligible needs of service users and their carers are met. This role offers opportunities to develop a variety of skills and knowledge related to community work with adults, with an emphasis on continuous professional development within a supportive team. Regular supervision, support, and induction will be provided.
The team currently operates Monday to Friday, with potential future extension to a 7-day service. The ability to travel across a wide geographical area is necessary, and a mileage allowance is payable.
